Life Member Application Form

I wish to apply for the Life Member designation with the PGA of Canada. By submitting this application form I am indicating that to the best of my knowledge:

  • I have been a Class “A” member of the Association in an active capacity for a minimum of twenty-five years
  • My age plus active years as a Class “A” member adds up to ninety at minimum
  • Casual employment accepted – hired for specified periods that may not exceed 90 working days in one calendar year.  Casual employees work in a position having no specified schedule.  They have neither guaranteed hours per day or days of work per week; rather, they are contacted when work is available at which time they may elect to accept or decline the work opportunity. Less than 15 hours a week based on “Seasonal” employment.
  • I must pay my Association dues as billed by November 1 and that if my application is granted, I will receive a refund of the appropriate dues.

Your application will be reviewed to ensure you meet all of the Life Member criteria listed above and must be approved by the PGA of Canada.

  • Should you decide to come out of retirement, you understand it is your responsibility to notify the PGA of Canada. You will be placed in a membership category based on my employment, and your dues will be adjusted accordingly. 
  • Your membership category is subject to review on an annual basis by the PGA of Canada and if your employment situation changes you will be placed in a membership category based on my employment, and your dues will be adjusted accordingly.  
  • You understand tournament privileges for Life Members will be determined by the National Board for National Championships and within the Zone by the Zone Board for Zone Championships.
Agreement I understand that by submitting this form I am responsible for the information being accurate, acknowledge that I am no longer working if the golf industry on a full-time or part-time basis, and meet the casual employment definition as defined in the above listed Life Member criteria. Should I decide to come out of retirement, I understand it is my responsibility to notify the PGA of Canada. I will be placed in a membership category based on my employment, and my dues will be adjusted accordingly. Should the PGA of Canada office determine it is inaccurate, I may be in breach of the PGA of Canada's Code of Professional Practice and may be subject to disciplinary action.
